Birth Registrations
This is official registration of persons born within the boundaries of Ghana by the Registry. The registration of birth is done in the district in which the birth occurred and by the following persons; the father/mother and or both parents, the next of kin of either parents, occupier of the premises which the child was born, an adult present at birth or a care taker of the child.
PROCEDURES AND REQUIREMENTS
Infant Registration
- Parent of the child must go to the nearest BDR Office in the District where the birth(s) occurred
- Present evidence of birth(s) (i.e. maternal health record book)
- Provide bio-data to the birth event
Name of child
Date of birth (would have already been indicated in the maternal health record book)
Place of birth (would have already been indicated in the maternal health record book)
Father’s name, age, occupation, religion, nationality and educational background
Mother’s name, age, occupation, religion, nationality and educational background
NB: Any nationality provided must be accompanied with evidence (i.e. national ID, passport, naturalization certificate)
Late Registration
- Parent of a minor or in the case of an adult, himself must go to the nearest BDR Office in the District where the birth(s) occurred
- Present evidence of birth(s) (i.e. weighing card, baptismal certificate provided maximum five years after the applicant was given birth to)
- Provide bio-data to the birth event

Place of birth (would have already been indicated in the evidence of birth)
Father’s name, age, occupation, religion, nationality and educational background
Mother’s name, age, occupation, religion, nationality and educational background
NB: Any nationality provided for parents must be accompanied with evidence (i.e national ID, passport, naturalization certificate)
